Comments
This resolution is a forward looking summary of an emerging, patient-centered, holisitc care model. If providers and community organizations, as well as local governments and perhaps the state executive branch support it, patients will benefit from it's coordinating and systematizing of the complex and dis-articulated chronic care model that is current.
